Who are patient forums for?
Patients, Carers and people with an interest in the condition.

What are patient forums?
They are NOT support groups. They are for local people with a desire to improve local services for people like them.

Cancer Forum
This forum meets at Keyworth Primary Care Centre, Bunny Lane, Keyworth, Notitngham.
Aims
• To raise awareness and positively promote cancer services to the population of the Principia GP Practices.
• To respond to and ensure that the views of patients, public and carers are integral to cancer and end of life plans and practice within Principia.
• To bring a patient and carer perspective to the commissioning cycle within Principia, the Primary Care Trust and other members of the partnership.
• To influence cancer services across the East Midlands by membership of the East Midlands Cancer Network Partnership Group.
Objectives
• To actively promote public awareness of cancer and end of life services including financial support / social care support / carer support.
• To actively encourage and engage with the widest range of public, patients and carers relating to cancer and end of life services.
• To be a source for local information and public expression to influence cancer and end of life aspects of Principia policy and practice.
• To work closely with East Midlands Cancer Network Partnership Group and Greater Nottingham Cancer Forum.

Diabetes Forum
This forum meets at Easthorpe House, 165 Loughborough Road, Ruddington, Nottingham.
Aims
• To act as a commissioning special interest group for Diabetes services.
• To bring patient/carer perspectives to the commissioning cycle within Principia and the Primary Care Trust. 
• To respond to and ensure that the views of patients, public and carers are integral to diabetes plans and practice within Principia.
Objectives
• To raise awareness of diabetes and positively promote access to Diabetes services including education to the population of the Principia GP Practices.
• To actively encourage and engage with the widest range of public, patients and carers relating to diabetes services.
• To be a source for local information and public expression to influence diabetes aspects of Principia policy and practice. 

Mental Health & Well Being Partnership
This forum usually meets at the Rushcliffe Arena, Rugby Road, West Bridgford, Nottingham.
Aim
• Rushcliffe Mental Health & Wellbeing Partnership will raise awareness and promote positive mental health and wellbeing to the community of Rushcliffe by involving and working with a range of agencies, organisations and service users and carers.
Objectives
• To raise the profile of mental health & wellbeing.
• To avoid duplication and strengthen partnerships across Rushcliffe around mental health & wellbeing work.
• To share and promote good working practice across the area.
• To respond to and ensure that the views of service users and carers are integral to plans and developments within the partnership.

50+ Forum Health Group
This forum meets at Easthorpe House, 165 Loughborough Road, Ruddington, Nottingham
Aim
• To bring patient/carer perspectives to the commissioning cycle within Principia and the Primary Care Trust and to respond to and ensure that the views of patients, public and carers are integral to the plans and practice within Principia.
Objectives
• To raise awareness of Health within the 50+ population of the Principia GP Practices.
• To actively promote public awareness of health & social care issues.
• To promote Government, Department of Health & NHS consultations.
• To actively encourage and engage with the widest range of public, patients and carers.
• To be a source for local information and public expression to influence all aspects of Principia policy and practice. 

Stroke Survivors Group
This group meets at St.Paul’s Church Hall, Boundary Road, West Bridgford, Nottingham.
